Stem Cell Donation vs Blood Donation in India: What's the Difference?

A question that comes up increasingly often among people who have donated blood — or are thinking about it — is: What about stem cell donation? Is it the same thing? Should I register for that too?

The short answer: stem cell and bone marrow donation are related to blood donation in their purpose — both save lives through biological giving — but they are different processes, different registries, and different commitments.

Understanding both is worthwhile for anyone who takes the idea of being a voluntary donor seriously. Because while blood donation saves three lives per unit, a single stem cell donation can be the only possible cure for a patient with blood cancer — and the matching challenge makes India's need for stem cell donors particularly urgent.


Blood Donation: The Foundation

Blood donation — whole blood, platelets, or plasma — provides immediate support for patients who need blood components right now. A trauma patient needs blood within the hour. A dengue patient needs platelets this week. A thalassemia child needs red blood cells this month.

Blood donation does not cure these conditions. It manages them — often brilliantly, often for years or decades. A thalassemia child who receives monthly transfusions can live a near-normal life. But the underlying genetic condition remains.

For some blood diseases, there is a cure that goes deeper than transfusion.


What Stem Cell Donation Does Differently

Haematopoietic stem cells — found in bone marrow and peripheral blood — are the cells from which all blood cells are made. Red blood cells, white blood cells, platelets: all originate from stem cells in the bone marrow.

In diseases where the bone marrow produces abnormal or dysfunctional blood cells — leukaemia, aplastic anaemia, certain lymphomas, and some inherited conditions — the entire blood-forming system needs to be replaced, not just supplemented.

A haematopoietic stem cell transplant (HSCT) does exactly this:

  1. The patient's diseased bone marrow is destroyed with high-dose chemotherapy and/or radiation
  2. Healthy stem cells from a compatible donor are infused
  3. The donor's stem cells migrate to the bone marrow and begin producing healthy blood cells
  4. If successful, the patient's blood system is essentially rebuilt from the donor's cells

For patients with acute leukaemia, high-risk lymphoma, or aplastic anaemia who have no matched family donor, an unrelated matched stem cell donor may be their only curative option.


The Matching Challenge: Why India Needs More Registered Donors

Stem cells cannot be given to just any recipient the way O-negative blood can. Stem cell compatibility is determined by HLA (Human Leukocyte Antigen) matching — a set of proteins on cell surfaces that the immune system uses to distinguish self from non-self.

For a transplant to succeed without life-threatening graft-versus-host disease (where the donor's immune cells attack the recipient's body), the HLA profile of donor and recipient must match at a high level. The best matches come from siblings — but only about 30% of patients find a matched sibling donor.

The other 70% need an unrelated donor from a registry.

The problem: HLA types are determined by genetics, and HLA variation follows ethnic patterns. People from diverse ethnic backgrounds need donors of similar ethnic backgrounds for the closest matches. India's extraordinarily diverse ethnic, linguistic, and regional population means that Indian patients with blood cancer often have very poor match probability in registries dominated by European or East Asian donors.

When an Indian patient with leukaemia needs a stem cell donor, the probability of finding a match depends on how many South Asian donors are registered globally.

How to Register as a Stem Cell Donor in India

Registering as a potential stem cell donor does not mean committing to donate right now. It means entering a registry that may match you with a patient — possibly years from now — who needs your specific HLA profile.

DKMS India registration:

  1. Visit DKMS India's website or find a registration drive
  2. Receive a swab kit — a cheek swab that you collect yourself at home
  3. Return the swab kit to DKMS India's laboratory
  4. Your HLA type is determined from the swab sample
  5. Your profile is entered into the global DKMS registry

If a patient anywhere in the world matches your HLA profile and needs a stem cell donor, you will be contacted.

Eligibility for stem cell donor registration:

  • Age 18–55 years
  • Generally healthy
  • Weight 50+ kg
  • Not pregnant

What Happens If You Are Called as a Match

Being a registered donor does not obligate you to donate if called. But most people who register understand that if they are called, it is because they may be the only person in the world who can save a specific patient's life.

There are two methods of donation:

Peripheral Blood Stem Cell (PBSC) donation — the more common method (about 80% of cases):

  • Growth factor injections (G-CSF) are given for 4–5 days before donation to stimulate stem cell release into peripheral blood
  • An apheresis machine then collects stem cells from peripheral blood — similar to platelet apheresis but collecting a different cell type
  • The process takes 4–6 hours on the donation day
  • The donor may experience bone pain or flu-like symptoms during the growth factor injection phase
  • Recovery is typically complete within 1–2 weeks

Bone Marrow Donation — the traditional method (about 20% of cases):

  • Performed under general anaesthesia
  • Bone marrow is extracted from the posterior iliac crest (lower back area of the pelvis) using needles
  • The donor is typically in hospital for 1–2 days
  • Soreness in the lower back for 1–2 weeks; full recovery within 3–4 weeks
  • Bone marrow fully replenishes within 4–6 weeks

The specific method needed depends on the patient's condition and the transplant team's recommendation. The donor is consulted and informed before any commitment to proceed.


Who Registers for Stem Cell Donation in India

DKMS India has featured several Indian donor stories that illustrate the breadth of people who register. Navya's story — "It Started with a Swab. It Led to Saving a Life" — illustrates the scale of what stem cell donation means from the recipient's side: a patient whose survival depends on a stranger's HLA type, a stranger who registered and was found.

The Indian leukaemia and blood cancer community is growing in awareness of stem cell donation, and young professionals — particularly in India's educated, health-conscious urban population — are increasingly registering through DKMS India swab campaigns.


How Blood Donation and Stem Cell Donation Work Together

They are not competing causes. They address different needs in the same patient population.

A leukaemia patient undergoing induction chemotherapy before transplant needs blood and platelets — urgently, repeatedly, through voluntary blood donors. After their stem cell transplant, during the engraftment period when their new bone marrow is establishing itself, they again need multiple blood component transfusions from voluntary donors.

The stem cell transplant itself comes from a matched stem cell donor.

Both forms of giving are needed for the same patient, at different points in their treatment.

Registering as a stem cell donor does not replace blood donation. For most people, the right answer is both: donate blood regularly, and register as a stem cell donor if you are under 55.
