A 2025 study from Rajarshi Dashrath Autonomous State Medical College and KGMU Lucknow examined blood donation knowledge among donors in Uttar Pradesh. The conclusion: persistent myths, inconsistent motivation, and limited institutional trust remain the dominant barriers to voluntary blood donation in India — even among people who have already donated once.
Myths are not trivial. In a country where one million units of blood are needed beyond what is currently donated, myths about blood donation directly cost lives. Every person who does not donate because they believe something false about blood donation is a unit that goes uncollected, a patient who goes without.

The myth: Many Indians believe that donating blood causes a lasting reduction in physical strength, energy, or health that does not fully recover.
The science: Blood donation causes a temporary reduction in blood volume that your body replenishes within days (plasma) to weeks (red blood cells). There is no permanent weakness associated with blood donation in healthy donors. Studies of regular donors — including centurion donors who have given 100+ times — show no evidence of long-term health impairment. The fatigue some donors feel in the 24–48 hours after donation is normal and temporary.
The myth: The needle used for blood donation can transmit blood-borne infections to the donor.
The science: All blood donation equipment — needles, collection bags, tubing — is single-use and sterile, opened from sealed packaging in front of the donor and disposed of immediately after. You cannot be infected by a needle that has only ever touched you. This is physically impossible with single-use equipment. Infection risk from blood donation for the donor is zero when performed at a licensed blood bank.
The myth: Donating blood creates a physical dependency that forces you to keep donating.
The science: There is no physiological mechanism through which blood donation could create physical addiction. You can stop donating at any time without any withdrawal or harm.
The myth: Without meat in the diet, iron levels are too low to donate blood.
The science: A study of iron status in regular voluntary blood donors found no significant difference between vegetarian and non-vegetarian donors for those who had donated fewer than 20 times. India's plant-based diet — rich in dal, rajma, sesame, leafy greens, and ragi — provides adequate non-heme iron for most vegetarian donors. Pairing iron-rich plant foods with Vitamin C significantly improves absorption. Most vegetarians in India can donate blood successfully, provided they eat a balanced diet and have adequate haemoglobin on the day of donation.
The myth: Blood donation is unsuitable for women due to menstruation, hormonal differences, or physical fragility.
The science: Women are fully eligible to donate blood between ages 18 and 65, provided they meet the same basic criteria as men (haemoglobin 12.5+ g/dL, weight 45+ kg, generally healthy). The donation interval for women is 120 days rather than 90 days — slightly longer to account for menstrual iron loss. Women cannot donate while pregnant or in the immediate postpartum period. There is no other gender-based restriction. India's blood donation gap is a cultural and social problem, not a physiological one.
The myth: Menstruation disqualifies a woman from blood donation.
The science: Women can donate blood while menstruating, provided their haemoglobin meets the threshold (12.5 g/dL) and they feel well. There is no medical rule prohibiting donation during menstruation. Women who feel unwell during their cycle may choose to wait a few days — but this is personal preference, not medical requirement. The haemoglobin check before each donation protects against the compounded iron loss concern.
The myth: Blood donation is a one-time gesture or annual ritual.
The science: Men can safely donate whole blood every 90 days (four times per year) and women every 120 days (approximately three times per year). The body fully recovers between donations at these intervals. India's blood shortage is substantially a frequency problem — most donors give once and never return. If every current donor came back every 90 days, the shortage would close without recruiting a single new donor.
The myth: Only physically large or robust people can donate blood safely.
The science: The minimum weight for blood donation in India is 45 kg. If you weigh 45 kg or more, are at least 18 years old, and meet other basic criteria, you can donate regardless of your height, build, or perceived physical size. The donation volume (350–450 ml) is scaled to body weight — donors below 60 kg typically give 350 ml rather than 450 ml.
The myth: Being on antihypertensive medication automatically disqualifies blood donation.
The science: Most antihypertensive medications are fully compatible with blood donation. What matters is whether your blood pressure is controlled on the day of donation (systolic ≤180 mmHg, diastolic ≤100 mmHg). If your readings are within these limits, donation proceeds regardless of whether you are on medication. Always disclose your medications at screening — in the vast majority of cases, antihypertensives are cleared.
The myth: Donating blood weakens your immune system, making you more susceptible to infections.
The science: Blood donation does not meaningfully affect immune function in healthy donors. White blood cells — the cells responsible for immune defence — replenish very quickly after donation (within hours to days). There is no documented evidence that blood donors have higher rates of infection than non-donors. You do not need to avoid sick people or take special precautions against infection after a standard blood donation.
The myth: Only young people can donate blood; age disqualifies older adults.
The science: Blood donation eligibility extends to age 65 in India under NBTC guidelines. Between 18 and 65, age alone is not a disqualifying factor. What matters is health status — a 60-year-old in good health with normal haemoglobin and blood pressure is an excellent donor. Many centurion donors have continued donating into their 60s.
The myth: Each blood donation permanently lowers your haemoglobin level, progressively making you anaemic over time.
The science: Haemoglobin drops temporarily after donation and returns to baseline within 3–4 weeks in healthy donors maintaining a good diet. The body's iron stores are fully replenished within 4–8 weeks. Regular blood donors who maintain iron-rich diets do not develop progressive anaemia. Haemoglobin is checked before every donation — donors who are borderline at any visit are deferred until their levels recover.
The myth: If you donated once 5 years ago and then donate again, the gap makes it dangerous.
The science: There is no harm in donating after a long gap, provided you currently meet all eligibility criteria. The eligibility criteria are assessed fresh at every donation visit — haemoglobin, blood pressure, recent health history. A person who donated 10 years ago and has been healthy since is evaluated exactly the same as a first-time donor.
The myth: Blood type determines personality, athletic ability, or compatible relationships — common in some cultural traditions.
The science: Blood type (ABO and Rh) is determined by antigens on red blood cell surfaces. These antigens have no known effect on personality, cognitive ability, athletic performance, or interpersonal compatibility. The scientific evidence for blood type personality theories is non-existent. Blood type matters for transfusion compatibility — and only for that.
The myth: Frequent donation strains the heart, kidneys, or liver.
The science: The 90-day interval for whole blood donation is set conservatively to ensure full recovery of iron stores and red blood cell counts. Studies of regular donors, including those who give four times per year for decades, show no evidence of cardiac, renal, or hepatic harm from donation at recommended intervals. The cardiovascular system adapts efficiently to blood volume changes that occur with donation. Some research actually suggests cardiovascular benefits for regular donors through iron reduction and haemostatic effects.
Each of these 15 myths stops real people from donating real blood. In a country that needs 14.6 million units per year and currently falls one million short, the myths do measurable harm.
