India's national blood donation statistics tell one story. The state-by-state data tells a very different one.
Nationally, voluntary blood donation reached 74.55% of all collections in 2024. That number sounds reasonable — until you look underneath it and discover that it is an average of extreme variation. Some Indian states have built voluntary donor cultures that match the world's best. Others are so far behind that patients in those states face genuinely life-threatening blood shortages that their neighbours do not.
The gap between Tripura and the states at the bottom of this list is not a minor statistical difference. It represents millions of patients receiving blood from safer, more reliable voluntary donors — or from the riskier replacement donor system that India has been trying to move away from since 1998.

For more than eight consecutive years, Tripura has led India in voluntary blood donation — a fact remarkable enough that it bears repeating: a small northeastern state of approximately 4 million people has consistently outperformed every other state in the country on voluntary blood donation rates.
The figures from different data points range from 93% to 97.3% voluntary donation — the highest in India. During 2015–16, the state's Health Minister reported that over 97% of blood collected was through voluntary donation, with a stated target of 100%.

Tamil Nadu's medical college system is among India's most active for blood donation drives. West Bengal has historically been among India's leading voluntary donation states — at one point holding first or second place nationally. The state's rate of 85.1% voluntary donation reflects a long tradition, particularly in Kolkata, of community blood donation drives.
West Bengal's Federation of Blood Banks in Kolkata has coordinated one of India's most active inter-bank networks. The state's leftist political tradition of community mobilisation has historically lent itself well to voluntary public health drives.
However, a study by the Association of Voluntary Blood Donors' Forum noted that West Bengal had been "outranked by Tamil Nadu" in recent years — suggesting some erosion in the state's relative position even as absolute numbers remain high.
Punjab and Maharashtra round out the top five. Maharashtra's position reflects the state's exceptional blood banking infrastructure (highest by volume and infrastructure indices nationally) and a long history of organised voluntary donation through IRCS, Red Cross, and private institutions.
Punjab's high voluntary rate is notable given that the state does not have the same density of large urban blood banking institutions as Maharashtra — suggesting stronger community-level voluntary culture driving the number.

PMC research documented that the VBD (voluntary blood donation) rate in almost 13 states of India is less than 50%. These states include many of the largest by population, concentrated in northern and northeastern India.
The eight Empowered Action Group (EAG) states — Bihar, Chhattisgarh, Jharkhand, Madhya Pradesh, Odisha, Rajasthan, Uttarakhand, and Uttar Pradesh — have the most significant access problems, with a BMJ Global Health study finding only 26% of residents within 30 minutes of a blood bank and median stock of just 0.6 units per 1,000 people.
Manipur has been cited as one of India's lowest performers on voluntary donation rates — the inverse of its northeast neighbour Tripura's exceptional record.
What drives low voluntary donation in these states?
1. High replacement donation dependency: In states where the healthcare system routinely asks patients' families to "arrange blood," the voluntary culture never develops. Replacement donation is the path of least resistance for both institutions and individuals.
2. Low awareness and high myth prevalence: PMC research in rural Bihar found significant gaps in blood donation knowledge and high rates of myth-based deterrence in the population.
3. Geographic and infrastructure barriers: More than 80% of PHCs in India lack blood storage. In states where blood banks are sparse and distant, casual voluntary donation is simply impossible for most people.
4. Anaemia rates: States with the highest anaemia prevalence in the eligible donor population also have the highest deferral rates — creating a painful cycle where those most affected by healthcare gaps are also least able to contribute to solutions.
5. Low institutional presence: The NGOs, medical college NSS programmes, and corporate CSR drives that drive voluntary donation in Bengaluru and Mumbai are largely absent in smaller cities across northern India.
The difference between 95% voluntary donation (Tripura) and below 50% (multiple northern states) is not just a number. It translates directly into patient outcomes.
Voluntary donors have significantly lower rates of transfusion-transmitted infections than replacement donors. A 15-year NAT study at Max Healthcare found that 98.68% of seropositive donations came from replacement donors — meaning the blood patients receive in high-replacement-donor states carries materially higher infection risk.
Blood deserts — areas where timely transfusion is logistically impossible — are concentrated in states with low voluntary donation. This means a road accident victim or obstetric emergency patient in Bihar faces not just a longer wait for blood, but less safe blood when it arrives.

3. Sustained infrastructure investment: Regular camps, mobile collection units, accessible blood banks, quality donor experiences
4. Visible role models: Centurion donors celebrated publicly, government employees and security forces leading by example, women donors highlighted rather than marginalised
5. Long-term thinking: Gujarat Red Cross's National Trophy record was built over 25+ consecutive years. Tripura's leadership was sustained for 8+ years. These outcomes are not the product of single campaigns — they are the product of cultural commitment that compounds over time.
